The offices of Desert West Obstetrics & Gynecology are open:
As a courtesy to other patients, we request office visit cancellations be called into the office at least 24 hours in advance.
Billing Questions
All billing questions should be directed to our Billing Department at 602-978-1500 ext. 4885. The office is open Monday through Friday from 8 a.m. - 4 p.m.
What to Bring
You will need to bring the following items to your visit:
You can save time by printing and completing the new patient paperwork prior to your first visit. Bring them with you to your first appointment.
Desert West physicians are members of the Banner Thunderbird and/or Banner Estrella medical staffs.
Privacy

- Friday from 7:00 am – 3:30 pm

- Driver’s license or other photo identification
- List of current medications and dosages
- Any pertinent medical records or test results
- Co-pay

- New GYN Patient Registration Form (Not Pregnant)
- New OB Patient Registration Form (Pregnant)
- Medical Records Release Form
